THE ROLEAs a GFH Patient Outreach Specialist, you'll conduct structured phone-based interactions with patients on behalf of our partner healthcare organizations. Using our software, you'll follow evidence-informed protocols to check in on patients, complete assessments, and escalate concerns when needed, all from a quiet space of your choosing.
